You wouldn't happen to be running on a Macintosh with its default sampling rate set to 96khz would you? I remember a problem with my Mac when I first got it where I had audio troubles all over the place (for example, no sound in MAME) until I went into the control panel and set the sound preferences to 48khz instead of 96khz.
